resurfacing concrete
what products would you recommend for creating a sloped surface on top of existing slab-on-grade for drainage purposes. slope approx 0.5% with
maximum thickness 2", minimum 1/2". area to be covered approx 10,000
square feet. area subject to forklight traffic, minimum compressive strength of 4000psi.

you say nothing about the existing slab:  thickness, condition, joint pattern(s), interior or exterior, cracks, etc.
there are certainly many topping products out there, but they can get expensive.  once you get above 1.5" thickness you might be able to get away with an ordinary pea stone mix.
the key to a quality topping is surface preparation.  i strongly recommend shot-blasting to get down to good sound clean concrete.  fog the surface until ssd, then follow with a brushed-in slurry of portland cement & water, keep damp by fogging until the topping material is placed.  match all existing joints in the topping.  choose your curing method carefully.
your slope of 1/16" per foot (0.5%) will likely be difficult to achieve consistently thoughout and may result in some areas that will be slow to drain.  i would recommend a bit more slope if chloride penetration or scaling may be of concern.
i'm sure others will have more details to add once you provide more data on the existing slab.
