cracked and uncracked concrete!
what is the technical definition of cracked and uncracked concretes? under what circustances do we design cracked concrete or otherwise?

a good concrete text book, like macgregor, along with a course in concrete will answer both of those questions.
1. all concrete inevitably cracks, large or small, coarse or fine. a crack is a crack, what "definition" are you exactly looking for?
1. if you are asking design of the concrete for flexural, and using strength design method, then the tension side of the concrete, at least on theory, is already cracked.
2. if you are concerned with leakage through cracks, see aci ch10.? for crack control criteria, and commentary on deviation of the "z" factors.  

a crack happens when the tension stress is greater than the modulus of rupture (typically around 7.5(f'c)^.5.  thisis obviously for flexural cracking.
is this for design of anchor bolts per appendix d?  or are you doing deflection calculations?  for appendix d, in certain seismic regions all concrete is assumed cracked.  so it all depends.
