lately there have been talks on a pca publication related to achor bolt design.
there is a very good design procedure outlined in the uniform building code, 1997 under section 1923.
also, some of the concepts and principles can be found in a publication by nelson stud welding division.
personally, i use the aisc lrfd bolt design under combined loading to design anchor bolts. as far as the anchorage strength of the concrete, i apply the 1997 ubc provisions as well as the provisions in the latest pci design handbook.
